How health coaching training can support personalised care
The NHS have committed to delivering a personalised care approach which aims to provide people with choice and control over the way their care is delivered. Helping people to gain the skills and knowledge needed to create their own personalised care plan will empower them to achieve their health and wellbeing goals. To support the delivery of personalised care within the community, three key new roles have been set up by NHS England; Health and Wellbeing Coach, Social Prescribing Link Worker and Care Coordinator.
Health coaching is a supported self-management intervention and is part of the NHS Long Term Plan’s commitment to make personalised care business as usual across the health and care system.
Health coaching skills are an integral part of the toolkit for Health and Wellbeing Coaches, Social Prescribing Link Workers and Care Coordinators.
