The Care Coordinator works closely with primary care professionals to identify and manage those with long term health conditions who are in need of proactive support. Those in the role will need the skills to build relationships and trust with the people they work with, supporting people in preparing a personalised care and support plan which they feel invested in. Care Coordinators work with people to encourage them to self-manage their own care as much as they can.
These roles are about working with individuals to build a trusting partnership and uncover the issues that are impacting their lives, identify what matters to them, and then create the connections needed to support them to engage with their health and wellbeing goals.
Our Programmes for Care Coordinators
We offer a 2 day Core Skills in Health Coaching programme or a 4 day Accredited Skills in Health Coaching programme, both are suitable for those working in the Care Coordinator role.
Our programmes provide care coordinators with the opportunity to develop a broad understanding of the principles of health coaching and behaviour change, while providing a practical set of tools that they can apply immediately in their work.
The programmes are accredited by the Personalised Care Institute and meet the standards and requirements described in the NHS England and NHS Improvement Health Coaching Implementation and Quality Summary Guide. Our four day programmes is also accredited by the European Mentoring and Coaching Council
We provide training for organisations and for individuals, through our Coaching Academy. Organisational training can be tailored to meet a client's specific needs. Programmes for individuals run each month and are ideal for anyone working in a personalised care role.
