The NHS have committed to delivering a personalised care approach which aims to provide people with choice and control over the way their care is delivered. Helping people to gain the skills and knowledge needed to create their own personalised care plan will empower them to achieve their health and wellbeing goals. To support the delivery of personalised care within the community, three key new roles have been set up by NHS England; Health and Wellbeing Coach, Social Prescribing Link Worker and Care Coordinator.
Our Services To Support Personalised Care
TPC Health have a range of programmes that support health care systems to embed a personalised care approach within their teams. Our programmes are suitable for practitioners from all health, care and voluntary sectors and are delivered by experienced facilitators who have an unrivalled understanding of how to develop people to use personalised care approaches.
Our health coaching programmes are approved by the Personalised Care Institute and our Accredited Skills programme is accredited by the European Mentoring and Coaching Council at Foundation level.
Our range of programmes are highly interactive with a focus on skills practice and development. We challenge participants to identify how they can apply their learning to their work place setting. Our health coaching programmes are followed up with support materials.
