Personalised Care Skills Development

Programmes to support health and social care organisations establish a personalised care approach

Providing your workforce with the knowledge, skills, and confidence to deliver care that is compassionate, effective, and truly personalised.

TPC Health has a long history of providing evidence-based personalised care training, and working alongside practitioners and organisations to embed personalised care within their teams, services, and day-to-day conversations. We are passionate about supporting and inspiring health and care professionals to deliver care that considers an individual’s strengths, needs, and expectations to provide the right care for them.

Personalised care supports individuals to make decisions about managing their health based on what matters to them, in collaboration with their healthcare professionals. It benefits the whole population, from those with long-term conditions and complex needs, to those managing mental health issues or social issues affecting their health and wellbeing. It addresses the limitations of a one size fits all health and care system that is unable to meet the growing complexity of people’s needs and expectations.

We have a range of programmes that support health care systems to embed a personalised care approach within their teams. Our programmes are suitable for practitioners from all health, care and voluntary sectors and are delivered by experienced facilitators who have an unrivalled understanding of how to develop people to use personalised care approaches

Our Personalised Care Skills Development facilitators have a minimum of 10 years’ experience in the healthcare sector and are practising health and wellbeing coaches. They all hold senior coaching and supervision accreditations with recognised bodies such as the EMCC, ILM and ICF as well as specific health coaching accreditations.

Having extensive experience of work in and with the health and care system our facilitators understand the psychological and practical challenges associated with working with people to enhance personalised care and enable behavioural change.

How does the TPC Health offer practical experience and follow up support?

All our programmes are interactive and take a hands-on format using experiential learning and skills practice to prepare participants to transfer their knowledge into practice and take action.

During each programme participants will have regular opportunities for practicing their skills with their peers during breakout sessions. To support embedding of learning we provide a post-programme call, regular learning e-shots, and access to our free monthly coaching CPD webinars.

What is TPC Health’s approach to evidence-based practice?

All our programmes are developed from evidence-based approaches, incorporating the latest research on behaviour change, psychology, and self-management.

This ensures the training and services you will receive are anchored in empirical and theoretical knowledge and ensures that you stand apart from those using unproven approaches. In addition, we support the EMCC core competencies and believe that coaching development should be grounded in this knowledge.
