Embed a more personalised, collaborative approach to care that is aligned with both clinical best practice and ethical standards
TPC Health has been at the forefront of supporting practitioners to deliver more personalised care to the communities they work in for over 10 years. We recognise that shared decision making is a fundamental component to personalised care and the challenges that practitioners and patients face in engaging in these types of conversations. Our programme supports practitioners to develop the knowledge, skills, and confidence to engage individuals in proactive conversations about what matters to them, and make decisions about their health and wellbeing.
Many training course focus only on the legal requirements and the policy behind shared decision making, and whilst this is obviously an important element to cover, feedback that we have received from practitioners is that they still don’t feel confident with what a good shared decision making conversation looks and feels like. We have therefore developed this programme to support practitioners to appreciate the value of shared decision making and their legal requirements, whilst also developing the necessary consultation skills and conversational frameworks to apply it. Our experiential sessions will also provide opportunities to practice real shared decision making conversations and receive feedback.
This programme was developed in partnership with NHS Birmingham and Solihull and NHS Herefordshire & Worcestershire, and is accredited by the Personalised Care Institute.

“The facilitators were very knowledgeable on topic and involved participants throughout training. Invited participants to share thoughts from own experience which made learning very applicable to practice. The role play on use of SDM option grid was very helpful”
“It was a well presented and very relevant programme, Im relatively “new” to working in my role in the community and I found it very meaningful and productive!”
“It gave me time to really think about how I have conversations with patients, and ensuring that I am giving them time to really discuss what is meaningful to them when making decisions As I am relatively new to my role I am still learning, and I need to be honest about that and become more confident in talking to the relevant members of the MDT to advocate for my patients The training today gave me confidence that team members are all keen to work collaboratively for the patients and in improving communication”

Informed decision making and consent in maternity and neonatal services
Ethical, professional and practical considerations The provision of high-quality personalised care is core to the 3-year delivery plan for maternity and neonatal services. Despite informed decision-making being an ethical, professional and legal standard, the 2023 maternity survey reported that only 76% of women felt as involved as they would have liked in decisions during childbirth…
Integrating Your Integrated Neighbourhood Team
The Journey of Integrated Care For nearly two decades, the UK has seen numerous national programmes aiming to integrate care for people living with long-term conditions (LTCs), especially those with multiple LTCs (MLTCs) or frailty.
