Our Personalised Care and Support Planning training emphasises the distinction between care plans and the care planning conversation, and how practitioners can develop core personalised care communication skills to build stronger relationships, understand what matters most to individuals, and address their care needs, while supporting them to take greater control of their health. Our highly interactive and practical courses are designed to strengthen your knowledge, skills, and confidence in these essential areas, so you can co-create an effective care plan based on what matters to people and improves patient outcomes.
Our training and support has been carefully developed to engage participants in the key mindset shifts which are fundamental to personalised care, alongside the practical conversational skills and frameworks to implement personalised care and support planning in practice. The training also focuses on identifying opportunities for how personalised care and support planning can be embedded into routine care, fostering a partnership approach between patients and practitioners.
This programme was developed in partnership with NHS Birmingham and Solihull and NHS Herefordshire & Worcestershire, and is accredited by the Personalised Care Institute.
The Programme Experience
Give your practitioners the necessary skills and tools to become communication experts
For leaders, managers or practitioners who are looking to understand the core principles and skills of personalised care and support planning so that they can learn to enhance the quality of their conversations.
Participants will learn the key behaviours, skills and techniques used in personalised care and how they can integrate this into their roles to improve their own performance and that of those they are supporting.
The programme is generally structured as 1 full day or 2 half dayworkshops, delivered a week or two apart to allow participants to practice their developing skills. Prior to the workshop participants are onboarded with a reflective briefing pack.
